Unit Specific Performance Objectives
Participates in the pre-screening and admission process that includes completion of the social history and identification of current care providers. .
Acts as the liaison between the program and community service providers regarding the individual‚s needs and in mobilizing community resources on behalf of the individual and his family.
Actively participates in treatment team meetings and advises the team regarding the impact of the individual‚s illness on the family and community service providers.
Accurate and timely chart documentation of all case management related activities. Coordinates court commitment procedures including preparation of relevant documentation, communication with mental health agencies, and providing testimony at commitment hearings as necessary.
Serves as the direct contact for referral and placement resources. Collaborates with the Family Intervention Specialist throughout the individual‚s admission regarding factors that effect disposition planning.
This includes planning and co-chairing case conferences and interagency meetings with family members and/or community service providers.
Collaborates with the Program Manager and Clinical Manager in the development and implementation of quality assurance measures.
Develops and implements the individual‚s transition planning and post-discharge follow-up. Performs other assigned duties.
Actively participates in own performance evaluation
Minimum Required Education/Experience Skills:
Minimum of a Bachelor‚s degree in an allied mental health or human services related field.
Minimum of two years experience as a mental health provider on a residential or related service unit for individuals who are dually diagnosed.
Comprehensive Crisis Management (CCM) certification must be maintained to include annual refresher. CPR certification renewed bi-annually or upon expiration of certification. Valid Act 33/34 clearances.
Valid driver's preferred Knowledgeable of the normal stages of development and behaviors; as well as the characteristics of individuals with developmental disabilities at those same stages.
Knowledge of Pennsylvania‚s Child Residential and Day Treatment Facility Regulations-55 Pa. Code, Chapter 3800.
Possess a general knowledge base of the DSM-IV-TR and its usage.
Maintains competence in identified universal, core and critical competencies.
Must meet job competencies within 90 days.
